How to Sign Up for Nagel Sports

STEP 1 – FinalForms Registration

• FinalForms | Parent Playbook
• Note: Blue Teams are Anderson HS District; Silver Teams are Turpin HS District
• Student must register for their high school district team. No Exceptions.

• Please  use Student Forest Hills Email when Registering. 

STEP 2 – Sports Physical

• OHSAA Physical Form
• REQUIRED PRIOR for Participation in any Nagel Practice or Tryout
• Sports Physicals - may be dropped off at Nagel Middle School Main Office during school hours OR preferably emailed to adenen@orthocincy.com

STEP 3 – Payment 

• Pay to Participate
• Please DO NOT PAY Fall fees until July
• Local Level Events Now Open for 2025-2026 Sports Participation Fee Payment
• Participation fees are due by the first official contest.
• Please pay for Tryout/Cut Sports AFTER your student is selected for the Final Roster.
